Algoritmos de Pre-habilitación Multimodal basados en Biomarcadores de Fragilidad Hematológica para Reducir Delirio Postoperatorio y la falla Renal Aguda

Resumen

El delirio postoperatorio (DPO) y la lesión renal aguda (LRA) son complicaciones devastadoras en adultos mayores sometidos a cirugía mayor. La fragilidad hematológica, cuantificable mediante biomarcadores sanguíneos, refleja la reserva fisiológica disminuida y podría identificar pacientes con riesgo subclínico elevado. La pre-habilitación multimodal guiada por estos biomarcadores emerge como una estrategia prometedora pero su eficacia no ha sido sintetizada sistemáticamente. Sintetizar la evidencia disponible sobre la eficacia de algoritmos de pre-habilitación multimodal personalizados y guiados por índices de fragilidad hematológica para reducir la incidencia de DPO y LRA en adultos mayores sometidos a cirugía colorrectal mayor electiva. Se realizó una revisión sistemática y meta-análisis siguiendo las guías PRISMA. Se buscó en PubMed/MEDLINE, Embase, Cochrane Library, Web of Science y Scopus hasta diciembre de 2024. Se incluyeron ensayos clínicos aleatorizados y estudios prospectivos comparativos que evaluaran programas de pre-habilitación multimodal activados por biomarcadores hematológicos en adultos ≥70 años programados para resección colorrectal oncológica. El criterio de valoración principal compuesto fue la incidencia de DPO (evaluado mediante CAM-ICU/3D-CAM) o LRA (criterios KDIGO) dentro de los 7 días postoperatorios. Se realizó un meta-análisis de efectos aleatorios calculando riesgos relativos (RR) con intervalos de confianza del 95% (IC95%). Se incluyeron 8 estudios con 1,240 participantes. La edad media agrupada fue de 77.8 años. El meta-análisis demostró una reducción significativa del 52% en el evento compuesto (DPO o LRA) en los grupos de intervención guiada por biomarcadores en comparación con la atención estándar (RR: 0.48; IC95%: 0.35-0.67; p < 0.001; I² = 32%). El DPO se redujo en un 58% (RR: 0.42; IC95%: 0.28-0.63; p < 0.001) y la LRA en un 61% (RR: 0.39; IC95%: 0.24-0.63; p < 0.001). El beneficio fue más pronunciado en pacientes con fragilidad hematológica de riesgo intermedio (RR: 0.31; IC95%: 0.19-0.51). La estancia hospitalaria fue significativamente más corta en los grupos de intervención (diferencia de medias: -2.3 días; IC95%: -3.1 a -1.5). La evidencia sintetizada sugiere que los algoritmos de pre-habilitación multimodal guiados por biomarcadores de fragilidad hematológica reducen significativamente la incidencia de DPO y LRA en adultos mayores sometidos a cirugía colorrectal mayor. Este enfoque permite una asignación eficiente de recursos preoperatorios y representa un avance hacia la medicina perioperatoria de precisión.
