Abstract

Subarachnoid hemorrhage (SAH) during pregnancy is an uncommon but potentially catastrophic neurological emergency, associated with high maternal and fetal morbidity and mortality. Its etiology is mainly related to the rupture of cerebral aneurysms and arteriovenous malformations, whose presentation is influenced by the hemodynamic and vascular changes inherent to pregnancy. In Ecuador, the high prevalence of hypertensive disorders of pregnancy, particularly preeclampsia and eclampsia, represents a major aggravating factor that increases the risk of hemorrhagic cerebrovascular events. This article aims to analyze the epidemiology, maternal and obstetric risk factors, clinical manifestations, diagnostic approach, therapeutic options, and maternal-fetal outcomes of subarachnoid hemorrhage in pregnant women, integrating recent international evidence and contextualizing it within the Ecuadorian setting. Early diagnosis, interdisciplinary management, and the strengthening of primary prevention through adequate prenatal care are emphasized.
