Oro-facial harmonization represents an emerging paradigm in contemporary aesthetic medicine, integrating dental and facial procedures to optimize the aesthetic balance of the lower and middle third of the face. The "Hollywood by Ciroi" protocol encompasses a therapeutic spectrum ranging from gingival surgery, placement of porcelain veneers and dental implants, to advanced facial contouring techniques such as submental liposuction, mandibular angle contouring, lip, chin and malar augmentation, nasolabial fold and eyelid fat pad filling, and botulinum toxin application. Objective: To describe and evaluate a comprehensive oro-facial harmonization protocol combining surgical and minimally invasive procedures to achieve natural, harmonious, and long-lasting aesthetic results, based on scientific evidence and anatomical principles. Methods: Prospective cohort study conducted between January 2024 and December 2025, including 120 patients (mean age 42.5 years, SD=10.3) undergoing the complete or segmented protocol according to individual needs. Aesthetic outcomes were assessed using patient satisfaction scale (0-10), 3D digital photographic analysis (Vectra system), evaluation by independent observers, and systematic recording of complications and recovery time. Statistical analysis included paired Student's t-tests and repeated measures ANOVA. Results: Overall satisfaction was 94.2% (mean score 9.2/10, SD=0.8). Combined procedures showed statistically significant synergy in improving the nasolabial angle (mean increase of 7°, p<0.01) and malar projection (mean increase of 4 mm, p<0.001). Complications were minor (5.8%), highlighting prolonged edema and transient dental hypersensitivity, with no severe vascular complications. Conclusions: The comprehensive Hollywood by Ciroi oro-facial harmonization protocol proves to be safe and highly effective, allowing personalized approaches that respect individual anatomy, ethnic characteristics, and patient expectations, with an exceptional satisfaction rate and favorable safety profile.
