Uso combinado de Reboa, Embolización y Fijación Precoz en Fracturas Inestables de Pelvis

Las fracturas inestables del anillo pélvico asociadas a hemorragia exanguinante constituyen una de las emergencias traumatológicas más letales, con mortalidades que alcanzan el 40-60%. El control hemorrágico precoz mediante estrategias combinadas podría reducir la falla multiorgánica y la necrosis de tejidos blandos. Objetivo: Evaluar la efectividad del uso combinado de REBOA (oclusión con balón de aorta), embolización selectiva y fijación interna precoz en la reducción de falla multiorgánica y necrosis de tejidos blandos en pacientes con fracturas inestables de anillo pélvico. Revisión sistemática y metaanálisis realizados siguiendo los lineamientos PRISMA 2020. Se buscó en PubMed/MEDLINE, Scopus, Web of Science, EMBASE y Cochrane Library (enero 2000-febrero 2026). Se incluyeron estudios que evaluaran estrategias combinadas de control hemorrágico en fracturas pélvicas inestables. La calidad metodológica se evaluó mediante ROBINS-I y Cochrane RoB-2. Resultados: Se identificaron 856 registros, de los cuales 32 estudios cumplieron criterios de inclusión (n=4,123 pacientes). La implementación de REBOA como puente a la hemostasia definitiva se asoció con reducción del 48% en requerimientos transfusionales (p<0.001) y disminución de la mortalidad por exanguinación del 38% al 19% (OR: 0.41; IC95%: 0.29-0.58). La combinación de embolización selectiva y fijación externa precoz redujo la incidencia de falla multiorgánica del 32% al 16% (p<0.001). La necrosis de tejidos blandos disminuyó del 24% al 9% con estrategias combinadas (OR: 0.32; IC95%: 0.20-0.51). El tiempo hasta la fijación definitiva <24 horas se asoció con menor estancia en UCI (11 vs 20 días, p<0.001). Se propone un algoritmo escalonado de manejo integrado. Se requieren ensayos aleatorizados multicéntricos para establecer la secuencia óptima de intervenciones. La estrategia combinada que integra REBOA como medida de rescate, embolización selectiva para control arterial y fijación interna precoz reduce significativamente la mortalidad, la falla multiorgánica y la necrosis de tejidos blandos en fracturas pélvicas inestables.
