Medicine cabinet review and management of expired medications: a challenge for ecopharmacovigilance

Abstract

The domestic management of medicines represents a critical aspect of both public and environmental health, due to the risks associated with drug accumulation, accidental consumption, and improper disposal of expired pharmaceuticals. To analyze these practices, a descriptive and cross-sectional study was conducted in 526 households using a structured questionnaire that collected information on the frequency of medicine cabinet inspections, the reasons for consuming expired drugs, and the types of storage employed. The results revealed that, although some households conduct regular inspections and use formal medicine cabinets, a significant proportion do not review their medications periodically and store them in improvised or unspecific places. Based on these data, a risk matrix was developed by cross-referencing the frequency of inspection with the type of storage to estimate the likelihood of consuming expired medications. The matrix indicated that households with formal storage and frequent inspection showed a low risk (approximately 5%), whereas those with improvised storage and no inspection exhibited a very high risk (up to 50%). Intermediate combinations of storage and inspection practices were associated with proportional risk levels (15–35%), demonstrating that the absence of preventive measures increases vulnerability to the consumption of expired drugs. These findings highlight the urgent need to consolidate integrated education and public health strategies aimed at promoting eco-pharmacovigilance, strengthening pharmacological safety in the home environment, and ensuring environmentally responsible drug management. Such actions are essential to advance toward a sustainable health model aligned with the Sustainable Development Goals.
