Structural Violence in Public Universities: The Case of Nursing Students in Clinical Practice Settings

Abstract

The study aimed to identify the prevalence and manifestations of different types of violence—physical, symbolic, harassment, and bullying—perceived by Nursing students during their clinical practice in public hospitals, as well as to analyze their emotional impact and the institutional dynamics that perpetuate them. A quantitative, descriptive, and cross-sectional approach was used through the application of a structured questionnaire with a Likert-type scale. The population consisted of students from the Bachelor’s Degree in Nursing at a Mexican public university, with a sample of 32 participants selected through convenience sampling. Most participants were women (over 80%), aged between 18 and 23, enrolled in the fifth semester, and performing their clinical practice in institutions such as IMSS, ISSSTE, the Women’s Hospital, and the José G. Parres General Hospital in Morelos, Mexico. The results show a higher presence of symbolic violence (average 1.02), followed by emotional impact (0.70), physical violence (0.62), bullying (0.39), and harassment (0.21). These findings indicate that the most common forms of violence are often invisible or normalized, linked to institutional hierarchies and gender inequalities. In conclusion, structural violence is manifested in clinical practice through power relations that affect students’ well-being and professional training. It is recommended to strengthen institutional prevention policies and promote a culture of respect, equity, and care in Nursing education.
