Towards a quality protocol for the prehospital environment: critical analysis of MSP management and auditing in Quito

Abstract

Prehospital care (PHC) is the emergency care provided at the scene of an incident, such as public roads or private residences, within the Quito metropolitan district. An analysis of audit and quality protocols at this level of care reveals a lack of specific audit protocols. Although regulations govern the hospital environment exist, their application at the PHC level is very limited. Therefore, this analysis was conducted to highlight the need to design clinical audit tools to bridge the gap between legal standards and their operational and technical implementation. Through a descriptive qualitative study, the regulations of the Ministry of Public Health (MSP) and the applicable regulations of the Agency for Quality Assurance of Health Services and Prepaid Medicine (ACESS) issued between 2020 and 2025 were analyzed. The results reveal a gap in clinical auditing and in the application of continuous improvement mechanisms in prehospital care. Therefore, it is imperative to propose the foundations for the development of an internal quality protocol for prehospital care that evaluates compliance with clinical guidelines, response times, and the final condition of patients, guaranteeing their care and safety with quality standards and continuous improvement.
